Think of your last dentist appointment and under which setting would you feel it is their responsibility to provide their home address. The address to write on receipts is where the consultation took place.<\/p>\n<p>An insurance company may request this information for quality control purposes, but insurers are bound by various legal constraints to maintain confidentiality.
